Data were obtained from a retrospective review of all patients who underwent cystectomy for bladder cancer at a single institution between 1981 and 1995. The median age was 66 years, and 69% of patients were men. Emotional distress as a result of cancer-related or other trauma was documented preoperatively. The median follow-up was 92 months. Of 87 eligible patients, 20% had been diagnosed as having a psychiatric disorder preoperatively, 17% had been in a relationship in which they had been subjected to prior psychological trauma, and 42% had experienced significant emotional distress as a result of their cancer. The 5-year disease-specific survival and disease-free survival were 46% and 31%, respectively. Significant preoperative psychological trauma was a strong predictor of bladder cancer recurrence (P = 0.005) and cancer-related death (P = 0.008). Patients who had experienced previous trauma also had a significantly higher incidence of disease recurrence (P = 0.03). Patients who developed psychological trauma because of cancer-related symptoms had a higher incidence of disease recurrence (P = 0.006). The disease-specific survival and disease-free survival of cystectomy patients with bladder cancer are less favorable than previously reported, even in patients without psychological trauma.
